Demineralized bone matrix(DBM)

생체의 골격을 지탱하는 뼈를 재생하기 위한 재료로 DBM이 언급되고 있다. 

 

이 재료는 1965년 Marshall Urist에 의해 개발되었고, 간단히 요약하여 말하자면,

 

뼈는 콜라겐과 미네랄의 조합으로 구성되어 있는데,

 

여기서 미네랄을 모두 제거하여 콜라겐 매트릭스만 남은 상태가 DBM이 된다.

다만, 미네랄의 제거에 의해 내부 구조를 지탱해주는 강성이 줄어들어, pore collapse 등이 일어날 수 있다. 

 

surface에서 일어나는 현상을 모사해서 사용하기 좋은 재료이며, 실제 재생이 필요한 뼈 손실 부위에 사용되기도 한다.

Demineralized bone matrix

Demineralized bone matrix (DBM) is allograft bone that has had the inorganic mineral removed, leaving behind the organic "collagen" matrix. It was first discovered by Marshall Urist in 1965 that the removal of the bone mineral exposes more biologically active bone morphogenetic proteins.[1] These growth factors modulate the differentiation of progenitor cells into osteoprogenitor cells, which are responsible for bone and cartilage formation. As a result of the demineralization process, DBM is more biologically active than undemineralized bone grafts; conversely the mechanical properties are significantly diminished.

Current clinical options

The success of a bone graft is determined by its ability to recruit host cells to the site of the graft and modulate their conversion into bone forming cells such as osteoblasts, to repair the defect. This will depend on the osteoconductive, osteoinductive and osteogenic capabilities of the graft.

Currently, autograft bone harvested from the iliac crest is considered the 'gold standard' due to its superior osteogenic properties. However, associated donor site morbidity, an increased surgery and recovery time, and a limited supply of donor bone are limiting its use.[2]

Allograft bone is a logical alternative to autograft. However, it must be rigorously processed and terminally sterilized prior to implantation to remove the risk of disease transmission or an immunological response. This processing removes the osteogenic and osteoinductive properties of the graft, leaving only an osteoconductive scaffold. These scaffolds are available in a range of preparations (such as morselized particles and struts) for different orthopaedic applications.

DBM has superior biological properties to undemineralised allograft bone,[3] as the removal of the mineral increases the osteoinductivity of the graft.

Q. DBM의 quality를 표준화 할 수 있는 지표들이 있는지.

 - Collagen structure section 방향에 따라 구조가 달라지는 건 확인되어 있음. 

 - Quality; 제조자에 따라 차이가 있을 수 있음, material origin에 따른 차이는 크지 않을 것으로 판단 된다고 함.

 - Commercialization 할 때, 필요한 내용이나, 연구실 수준에서 고려하기는 힘든 주제인 것 같다.
